A Therapeutic Daytime Program For Older Adults
Living Independently, As Well As Respite Care For Their Caregivers.
PEP is excited to have now opened a Seniors Day Program in Smiths Fall. The Smiths Falls Program will mirror the Perth Program but will only run on Thursdays.

PEP operates Monday, Wednesday, Thursday and Friday from 9:30am till 3pm PEP in Perth (Elliot Street Clinic) and one day a week in Smiths Falls (91 Cornelia Court, Suite 110). PEP offers a therapy centred program for it’s participants which will include socialization, nutritional assessment and instruction, physical assessment and activation, cognitive assessment and activities which will improve function, and emotional stimulation and support.
Detailed aspects of our program include exercise and fall prevention training, health optimization interventions, advocacy for seniors, medication monitoring and family caregiver leave. Participants will receive nutritional mid-morning and afternoon snacks, as well as a three course lunch each day they are at the program. Intake as well as ongoing assessments will guide program staff to work towards outcome goals set with participants, family and healthcare providers.
Three full time health professionals work within the program, along with trained volunteers from our community.
In addition to the direct benefits to participants, PEP will afford family caregivers a respite from the demands of full-time care giving for someone who needs supervision, as well as educational, prognostic and social work assistance.
Each day has its own roster, and participants attend the same day(s) and times each week. Most people attend one to two days per week. Additional hours and days may be arranged, depending on caregiver needs and the ability of staff to provide the needed care.
The PEP program was something of a lifeline for my mother, who otherwise didn’t get out much. She always craved social engagement, but had a hard time initiating it on her own. PEP took the pressure off, and she loved it: she updated me on the activities every week. ~ W.D.
The Day Program Has Two Primary Goals:
(1) to enhance or maintain the functioning level (social, physical, emotional and cognitive) of older adults on an individual basis in concert with input from their healthcare providers and family
(2) to provide respite, encouragement for the family / caregivers of our participants

- Lunch and snack ( hot lunch)
- Assistance with, or supervision of, eating, walking, toileting
- Physical Therapy/activation
- Art Therapy
- Music Therapy
- Socialization and peer support
- Social work networking
- On-site nurse
- Disease-specific activities
Who Is The Program Geared Towards?
PEP is a program for the older adult, living independently, with multiple chronic diseases. The participant will typically be frail, is beginning to be unable to perform activities of daily living – impacting their quality of life, and is often vulnerable to physical and/or mental illness.

Perth Enrichment Program for Older Adults ( PEP) is located in the Elliot Street Clinic at 12 Elliot Street in Perth.
What Is The Cost To Participate In The Day Program?
Cost per participant per day is $35.00 which includes a hot lunch.
How does one enroll in the program?
To enroll in PEP, simply fill in an application form, and have the participating individuals family physician or care professional submit a standard consult referral. Enrollment forms can be picked up at our program location, or can be faxed or emailed by request. They can be filled out and faxed to Executive Director Pauline Fitchett at (613)201-7172 – scanned and emailed to firstname.lastname@example.org – or brought in to the program location at 12 Elliot Street, Perth ON.
