Integrated Care for Older Adults and Their Families

Part of our mandate includes enhancing the older adult under our care by supporting their care-givers. Families can be educated on the nature of the condition their loved one is experiencing, and coached in how to respond to it. They will receive regular objective updates on the functionality of the patients as well as a better sense of the rate of change, leading to better planning. They will be tied into existing community groups and services via our experienced knowledge base.

Working in concert with participant’s attending physician, PEP’s program looks to optimize the health and well being of each participant. All medication, including over-the-counter drugs, must be medically prescribed and taken under direct supervision of a professional staff member.

    I’m a psychotherapist in Perth, supporting families facing life transitions, including coordinating care for older adults who are not eligible for nursing homes but who could benefit from more assistance than their families are able to provide on a day-to-day basis. I would be happy to mail you some of our brochures if you like as well. We run a Seniors’ Day Program but also a COPD Exercise and Education Program.
