How to enroll in PEP

• Simply fill in an application form, and have the participating individuals family physician or care professional submit a standard consult referral.
• Enrollment forms can be picked up at our program location, or can be faxed or emailed by request.
• They can be filled out and faxed to Executive Director Suzanne Rintoul at (613)201-5886 – scanned and emailed to [email protected] – or brought in to the program location at 12 Elliot Street, Perth ON
